When I first read the (very thorough and detailed) instructions, I was intimidated. I had gel polish on my big toes remaining that I wasn't willing to work to hard to remove at home, so I knew that could be a problem.
I think the reason people might find this process difficult is that not everyone has a lot of experience doing nails or getting their nails done. For the adhesive in the package to work you have to have a dry surface that has been roughed up a bit with a file, but free from that debris. The instructions say to soak your foot in order to fit the contraption, leave it for a bit, and then there is a very important blow drying step (you can air dry, but it has to get dry, that is the key). There's a step to rough it up with the provided nail file and then a very important alcohol swab step. This is required to get a good bond for the glue!
When it comes to actually gluing the brace on... it's tricky. For people with low mobility or who are inflexible, this can be very difficult to do on your own. You have to keep the brace very still after you've set the glue and hold it for a bit, and if you can't reach or get a good angle, you might not get the best bond.
The whole process took me 30 min by myself..I felt immediate pressure relief BUT I had severe toe pain for 1.5 days after. I'm sure this is because my toe was already pretty inflamed before I started, but if you think about it... your body is also filling a gap left by your toenail, so maybe that's also part of the pain? Anyway, after that initial time I WAS PAIN FREE AND IN BASICALLY 2 DAYS MY TOENAIL WAS FLAT AND CHILL. Honestly, I didn't know my left toe was supposed to look like that (see pic). I don't have a before pic, but it was red and angry and painful and similar to my right toe.
I'm going to do my right toe next. I wish I had done my left sooner.
Of note, about a week in for left toe, one of the sides came up because the gel polish underneath finally gave up. The fix was super easy, and I just put a little more glue down and held for a minute. Piece of cake and no pain.

I honestly don't know if it works for some people, but I can't recommend it based on my experience.

Changed my rating from 2 stars to 3 after they promptly offered a full refund.

The next challenge is to attach both sides of the retainer spring- not easy but certainly doable. It remains to be seen if this solves my issue temporarily or permanently, but I'm very happy with the initial result.
The process revealed a surprising "divot" in the side of my toe where the nail was eating into it. Not much inflammation but enough of a gouge that I've applied some Neosporin just to be safe. I'm curious to see how the included toe caps hold up over the next week or two with showers (and hot tub soaks)
Overall, I'm initially very satisfied with this despite the difficulties in using it. It performed as promised and avoided a doctor visit. Whether that will last or not remains to be seen.
I used the little screw device once to lift the sides of the nail and then used the little springs a few hours here and there. The pressure on the skin on both sides of the nail is gone. The redness is gone.
I will continue to use it until I can barely remember my toe had an issue.
It's worth the price for sure!
